Late Registration for the Fall 2008/ Spring 2009 Season  

Its not too late to register your child for the Fall 2008/Spring 2009 season.

Players who register after our last registration on March 13th  will be put on a wait list and be placed once teams are formed and openings can be determined.  There is no guarantee of team or location to anyone who registers late.

You can Pre-register your child on-line right here through the eAYSO registration system.  Then bring your players forms to the PCSL office.  Office hours are Tuesday and Thursday 9am-1pm.   

Parents who complete on-line pre-registration (and print 3 copies of the forms) from the convenience of home will move through the final on-site process more quickly. All players must be pre-registered on-line.  

Here's what to do to register for the Fall 2008/Spring 2009 season.

Review the Checklist for Pre-Registration:  

ü      Location code for neighborhood your child lives in (refer to location map see…..).  The location box on your form must be manually completed for the League to place your child on a team. Location Map (click here)

ü      Birth date of player and school your child attends.  

ü      Parent information complete in case of emergency.  Also be prepared with emergency contact & phone number other than your home (cell phone, neighbor or friend).  

ü      Medical conditions including allergies, physicians name & number in case of emergency.  

ü      Email address so that your coach or the League can reach you efficiently.  If you do not use email simply indicate that in the player email box so we know to telephone you.  

ü      Volunteer form requires personal and professional references.  

ü      After you register your player print 3 copies of the completed form.

Come to Registration Prepared with:  

Ø      3 copies per player of the completed pre-registration form signed by a parent or legal guardian.  

Ø      Birth certificate, passport or other proof of birth date if the player is new to the League.  We ask that you present this but, we do not need to keep a copy.  

Ø      Valid Drivers License or State ID of the parent/guardian signing up the child.  

Ø      Check book or cash for $105 per player registration fee.  No credit cards are accepted.  

Ø      Two copies of the on-line volunteer form, completed and signed for all participating volunteers.
